Proposed Orders
Forms:

LBR 9013-1 requires the use of the local form "Request for Entry of Order Granting Motion." This requirement applies to all requests for entry of proposed orders -- whether filed electronically or conventionally in both main case and adversary proceedings.

Main Case Forms:

If the attorney submitting the order is also certifying as to notice and the absence of objections, use LF9013.

If the attorney is not the person certifying as to notice and the absence of objections, use LF9013-2.

Adversary Forms:

If the attorney submitting the order is also certifying as to notice and the absence of objections, use LF9013AP.

If the attorney is not the person certifying as to notice and the absence of objections, use LF9013AP-2.


Order Layout Restrictions:

1) All proposed orders must be combined with the appropriate "Request for Entry of Order" form into one PDF file before filing in the CM/ECF system.

2) Do not include a line for the judge's signature. Our signature stamp includes a line under the signature.

3) Do not include "Dated this day…" language. Our signature stamp includes the date and time.

4) The proposed order must be signed by all required parties prior to submitting it to the court.

5) The signature block area on the last page of the order must be left blank. Dimensions of the signature block can be found here.

6) All orders must be formatted for 8.5" x 11" paper only.

Filing:

File proposed orders with the "Request for Entry of Order" form as one PDF file in the CM/ECF system.

Use the "File Request for Entry of Order and Proposed Order" event, located under the "Bankruptcy" menu. (Please note: Do not include the word "proposed" in the title.)
